As applicable, certain state privacy laws, such as the California Consumer Privacy Act of 2018 (“CCPA”) and the California Privacy Rights Act of 2020 (“CPRA”), the Virginia Consumer Data Protection Act (“VCDPA”), the Colorado Privacy Act (“ColoPA”), the Connecticut Act Concerning Personal Data Privacy and Online Monitoring (“CTDPA”), Utah Consumer Privacy Act (“UCPA”), provide their residents, respectively, with specific rights regarding their personal information. This form allows you, or someone authorized on your behalf, to exercise the rights that may be available to under the CCPA, CPRA, VCDPA, ColoPA, CTDPA, or UCPA to request (1) to opt-out of the sale of your personal information by DriveCentric, Inc. and its service providers (“Right to Opt-Out”); (2) for DriveCentric, Inc. to disclose what personal information it has collected about you (“Right to Know”); (3) for DriveCentric, Inc. to deletion personal information it has collected about you (“Right to Delete”); and (4) for DriveCentric, Inc. to correct inaccuracies in the personal information it has collected about you (“Right to Correct”). The Right to Correct is not afforded to residents of Utah.
